careers.

Fayetteville State University’s Upward Bound Program is a federally funded
program designed to provide academic support services and educational
opportunities to high school participants to assist them with preparing for college.
Prior to submitting your Upward Bound Application, please take a moment to
review the checklist below to ensure that you provide all the necessary
documentation. Incomplete applications will delay your possible admittance into
the program. We will notify you of our decision after receiving the completed
application.

• Completed Upward Bound Application

• Parent’s signed 2021 Tax Form (1040, 1040A, 1040 EZ)

• Unofficial high school transcript (9th grade applicants, please submit
  your current report card)

• Personal Statement

• Letter of Recommendation (optional)
