Consent Form for Participation and Release of School Records / Media Release
1. I authorize the Educational Talent Search Program of Tallahassee Community College to access and/or receive copies of my academic transcripts, grades, report cards, FSA scores, other test scores, promotion/retention status, financial aid acceptance, post-secondary enrollment status and any other academic information and test results as necessary.
2. I give permission for my child to participate in certain activities that require transportation provided by the program.
3. I will hold TRIO Educational Talent Search program and Tallahassee Community College harmless for any damage that might result from the activities and releases mentioned above.

I authorize Educational Talent Search Program to publish photographs, videos, and other media of my child on behalf of the program.*
