    Thank you for your interest in the Pima Community College Upward Bound Program. Upward Bound is designed to encourage students from low-income families and/or whose parents did not graduate from a four-year college to develop the skills and motivation necessary for success in college and beyond. Students accepted into the program are participants until they graduate from high school. All program services are free to participants. We expect students to make a serious commitment to both their academic education and the Upward Bound program. Please make sure that you complete the application and attach all required documentation (see below). All information provided on this application is confidential and kept in a secure place.

    For a complete Upward Bound application, please make sure you include the following:

    • Complete Application - Sign and Date
    • Two Letters of Recommendation (must be submitted by your high school teacher/counselor)
    • Student Questionnaire & Personal Statement
    • Uploaded copy of your social security card or parent(s) income verification
    • Copy of your U.S. birth certificate, U.S. passport, or Alien Registration Card
    • Unofficial copy of your transcript or most recent report card
    • Most recent standardized test scores (if available)

    • The personal essay is an important part of the selection process. In a 250 or more word essay (or in a two minute video/recording of yourself) discuss ONE of the following topics. Feel free to type your essay if you wish and attach additional pages if necessary.

      1) What is your purpose in applying to the Upward Bound Program? What do you believe you will gain from the experience, and what contributions can you make towards the program?

      2) What events or persons in your background have influenced you in your educational and career goals?

      FOSTER CARE NOTE: If student is in foster care or in the care of Division for Youth or other institution, the parental income information is not required. However, written verification of a student's status is required from the appropriate agency. Foster parents may sign the application.

    • In accordance with the TRIO Upward Bound eligibility requirements set forth by the U.S. Department of Education, I hereby certify and attest under penalty of perjury that the information I have provided on this application is true and correct to the best of my knowledge. I authorize Pima Community College Upward Bound Program access to my student’s academic and personal information through the school or district database and/or receive copies of transcripts, test scores, and any other information that may be necessary for participation in the program. I further authorize the staff of Pima Community College Upward Bound access to my student’s college enrollment, retention, and completion information, as well as financial aid information from colleges and the federal government. I further certify that I am aware that the provision of any false, fictitious, or fraudulent information or omission of any material fact may subject me to criminal, civil or administrative penalties for fraud, false statements, false claims, or other violations. (U.S. Code Title 18, Section 1001; Title 20, Section 1097; and Title 31, Sections 3729-20 and 3801-3812). 

      The following consent shall be valid for the entire duration of my son/daughter’s participation in Upward Bound and until completion of a bachelor’s degree, or for 6 years following graduation from high school. I hereby grant permission for my son/daughter to participate in the Pima Community College – Northwest Campus Upward Bound year-round programming. I also give my consent for the following:

      • For the high school where my son/daughter is enrolled to make available to the Upward Bound Program staff academic records (class schedules, test scores, transcripts, and attendance) 
      • Communication to obtain documentation verifying an applicant’s U.S. citizenship, or national permanent resident status, or of his or her intent to become a permanent resident.

    • The Family Educational Rights and Privacy Act (FERPA), 20 U.S.C. §1232g,.,  is respectively federal and state law  providing for the review and disclosure of student educational records. The Pima Community College Upward Bound. TRIO program  will not permit access to or the release of personally identifiable information contained in student educational records to any party without the written consent of the student, except as authorized by FERPA.
