    • Dear Parent/Guardian:

      Your child has expressed an interest in participating in the Educational Talent Search Program of North Carolina A&T State. ETS is funded by the U.S. Department of Education and sponsored locally by North Carolina A&T State University and provides free college, career, and financial aid information to high school students each year. ETS identifies students who demonstrate an interest or desire to attend college and provides them with the motivation and support to enroll in a program of postsecondary education (college, university, technical or vocational school) after high school graduation.

      Educational Talent Search encourages and prepares students for postsecondary enrollment by providing assigned students with the following services and activities through group workshops and individual appointments held at
      designated target schools:

      • Academic Advisement
      • College Awareness Activities
      • Career Exploration Activities
      • Financial Aid Awareness Activities
      • Scholarship Information
      • College & Career Field Trips
      • Individual Assistance with College Admissions, Scholarship Applications & Financial Aid Forms

      In order to be eligible for selection as ETS participants, students must meet the following criterion:

      • Be a U.S Citizen or Permanent Resident Alien;
      • Be at least 11 years of age;
      • Commit to remain in the ETS Project until HS graduation;
      • Demonstrate an interest or desire to attend college.

      Please complete this part of the application and return it to the address or fax number listed above. You may also return it to your child’s school counselor. All completed applications will be reviewed, and eligible applicants contacted within 14 days regarding their status.

      __ I authorize the Educational Talent Search Program of North Carolina A&T State University to access and/or receive copies of my student’s academic transcripts, grade reports, report cards, SAT/ACT scores, and any other academic information and test results necessary to complete the program’s application process. All the information received will be kept confidential in compliance with the Family rights and privacy act.

      CONSENT FOR ACTIVITY PARTICIPATION

      __ I grant permission for my student to participate in all activities sponsored by the TRiO Educational Talent Search program. I understand that the appropriate number of chaperones will accompany all trips, and students will be adequately supervised. I shall hold harmless NC A&T, its trustees, officers, employees and agents from any liability, losses, costs, damages, claims or causes of action of any kind or nature whatsoever, and expenses, including attorneys' fees, arising from or proximately caused by my child's participation in this program, including any travel.

      INTERNET/PHOTO RELEASE AGREEMENT

      __ My signature below indicates that I am providing consent for my son/daughter to utilize the Internet for educational purposes in accordance with the Acceptable Use Policy of NC A&T State University. Additionally, I agree and consent during TS activities to be placed on the Web and in public information materials. I hereby waive any right to inspect or approve the finished electronic, photograph, or printed matter that may be used in conjunction with them now or in the future.
