As the parent and/or legal guardian of the student listed above, I authorize and permit my child to participate in any and all academic year component activities (classes, tutorials, workshops, field trips, work study internship, meetings and events, etc.) sponsored and/or conducted by the Educational Talent Search (ETS) Program of the University of Southern California. I also give permission for my child to be transported between his/her high school, the University of Southern California campus, and the scheduled events when the ETS Program has scheduled events for its participants. I acknowledge the nature of such activities or trips may expose my child to hazards or risks that may result in his or her illness, personal injury or death, and I understand the nature of such hazards and risks.
I do hereby grant permission to the Educational Talent Search (ETS) of the University of Southern California and its authorized representatives, to furnish first aid as my son/daughter may require, as well as to seek medical attention through the nearest medical facilities when students are on field trips and other authorized activities. This permission is conditioned upon the understanding that in the event of serious illness or the need for hospitalization and/or major surgery, ETS will use all reasonable efforts to contact me. Failure in such efforts should not prevent ETS from providing emergency treatment as may be necessary for the best interest of my child.
Student School Records Release Authorization
I/We authorize Educational Talent Search (ETS) to obtain documents relative to and consistent with my child’s education. Such documents may include: a copy of my child’s school transcript, test scores, ACT/SAT or GED scores, and school lunch program eligibility. I/We authorize ETS to obtain documents related to my application for receipt of student financial assistance (federal, state, or other), a copy of my award notification from the financial aid office, and college admission information. I/We authorize ETS to obtain information from any agency or program providing supplemental services.
Media Publication Release Authorization
I hereby grant the ETS Project of the University of Southern California full and absolute permission and all rights to copyright, publish, display, and use for any legal purpose and all photographs, videos together with descriptive text or statements, in which I or my property or my child appear.
Please note that due to COVID-19, the California Stay-at-Home Orders, and LAUSD distance learning, all USC TRIO ETS services are temporarily provided virtually via Zoom video conferencing, email, and or phone (i.e. Google Voice), until further notice.
Parents/guardians are responsible to monitor the online activities of their children to ensure content is age appropriate and to ensure the safety of their children. For safety purposes, as recommended by USC Youth Protection and Programming, the USC TRIO ETS Program requires all virtual communication (i.e. video, chat, audio, text messages, emails, etc.) between USC TRIO ETS staff and students, to be recorded and documented.
This Program is 100% federally funded by the U.S. Department of Education. The personal information you give USC Educational Talent Search Program is for the U.S Department of Education. The information is protected by the Privacy Act. No one may see the information unless they are employed by USC Educational Talent Search. The information is necessary to determine if your child is eligible to participate in the Program and helps the U.S. Dept. of Education to measure his/her success. The U.S. Department of Education has the authority to gather such information (20usd1231a).
By signing below, I authorize/agree to Section 4 of this application and certify that the information on this application is accurate to the best of my knowledge.
