    UPWARD BOUND PROGRAM

    2021-2022 Program Application

  • Dear Student and Parent/Guardian:

    Thank you for your interest in the GECAC TRIO Upward Bound (UB) program. To be eligible for the program, a student must be currently in 9th, 10th, or 11th grade attending either Erie high school, Corry high school or the Charter School of Excellence, S/he must also meet modest income guidelines and/or be a first-generation college-bound student.

    Upward Bound is an academic program that requires a commitment from the student and cooperation from parents and family members. A student joining Upward Bound must actively participate in our after-school program during the academic year and are highly encouraged to participate in our six-week summer academy. Once accepted, we expect students to continue with the program for the duration of his/her high school career.

    Upward Bound is a federally-funded program; there is no fee charged for participation. Students receive a modest stipend based on attendance.

    At this time, all of our services are online/virtual; students will be provided with a GECAC TRIO Upward Bound issued Chromebook to participate in the academic year and summer academy. We will follow the state of Pennsylvania's guidelines and timelines for a safe return to group in person meetings and Saturday sessions.

    The following application forms must be filled out completely, signed, and the additional documentation requested submitted. Incomplete applications will be returned, which will delay your family interview and may jeopardize your child's chances of being accepted during the current admissions process. In addition to the application, we require the following documents:

    • COPY OF MOST RECENT SCHOOL TRANSCRIPT OR REPORT CARD (we will contact your Guidance Counselor for this)

    • 2 RECOMMENDATION FORMS FROM GUIDANCE COUNSELOR OR TEACHER (We will email the recommendation forms to two teachers you choose)

      FOR THE APPLICANT TO QUALIFY, VERIFICATION OF INCOME IS NECESSARY.
      ALL INFORMATION IS CONFIDENTIAL AND FOR PROGRAM USE ONLY

      Program eligibility is determined and partially based on, ANNUAL TAXABLE INCOME.

    • PART 5: PARENT COMMITMENT / RECORDS RELEASE 
    • I certify that the family and financial information is true, correct, and complete to the best of my knowledge. I hereby authorize the GECAC Upward Bound Program Staff to:

      • Access my son’s / daughter’s school or agency records to determine eligibility for the program, monitor his / her status and progress in secondary and post-secondary education and determine eligibility for financial aid, including scholarships and grants.
      • Access any of my son’s / daughter’s school health records when a medical condition has been disclosed.
      • Use my son’s / daughter’s name and /or picture in any newsletter, web page, social media, brochure, video or other public relations materials that pertain to the Upward Bound Program or GECAC.
      • Provide in-school academic support services and career exploration in order to assist my son / daughter with choices regarding education beyond high school.
      • Encourage my son / daughter to obtain before and / or after school tutoring services if needed, as offered by his /her school district or the Upward Bound Program to keep his /her GPA at or above a 2.5. 

      We understand that, if selected, our son / daughter will be required to attend and participate in meetings, activities and field trips on an ongoing basis, including two Saturday Sessions a month during the academic year.  He / she will also attend tutoring classes, if needed until his /her graduation from high school. If possible, he / she will participate in our 6 week summer academy at least one summer during his/her time in the Upward Bound Program.  This application remains in effect until my son / daughter and I notify the UB Program Director in writing, that he / she no longer wishes to participate in the Upward Bound Program.
