Southern Illinois University – Edwardsville Upward Bound Programs are federally funded through the U.S. Department of Education to provide educational support services, at no cost, to eligible students in the local school districts. Upward Bound is designed to provide the necessary supports so participants can develop the skills and mindset to complete high school, and to enter and succeed in a program of postsecondary education.
Southern Illinois University – Edwardsville may request access to the student records including standardized test scores, grades, attendance, behavior reports, and summer school enrollment for any student enrolled in their program. This would enable Southern Illinois University – Edwardsville to identify and assign appropriate supports and services to students. If granted access, Southern Illinois University – Edwardsville staff must maintain the confidentiality of student information, and not re-disclose the information to persons not identified in this consent. The Southern Illinois University – Edwardsville staff are only permitted to access student records in their own program and to the extent necessary to perform his/her duties. In addition, Southern Illinois University – Edwardsville may share information about his/her program with school district staff, in order to better serve students.
Confidential information may only be shared to the extent that the information is relevant to the student’s educational progress, safety, or well-being. Student information may be disclosed in a grave medical emergency which necessitates facilitation of medical care.
A parent/guardian authorization is required to allow Southern Illinois University – Edwardsville staff to access you child’s data. Please indicate your consent below.
I have read the above and consent to Collinsville Community Unit School District #10 to release, obtain, and exchange my child’s information from school district staff and Southern Illinois University – Edwardsville until my child reaches the age of nineteen years old.
