Dear Parent or Guardian,
Your child has been given the opportunity to participate in Educational Talent Search (ETS), one of the TRiO programs of the U.S. Department of Education. Our program’s mission is to help students succeed in school, explore career interests, and then link them to the education/training needed after high school that will help make their goals come true.
There is absolutely NO CHARGE to students or parents for Educational Talent Search services and materials. We believe that your child is eligible to receive our services. In order to confirm your child’s eligibility, it is necessary for you and your child to complete this form and return it to the school or to the ETS advisor. Upon remittance, ETS staff will review this application and notification will be given as to the student’s status for the ETS program. Because we are federally funded we must receive this information to confirm services are being provided to eligible students. THE ENTIRE FORM, INCLUDING ALL LINES, MUST BE FILLED OUT IN ORDER TO BE ADMITTED INTO THE PROGRAM. ALL INFORMATION IS KEPT CONFIDENTIAL.

Important Note about Grades:
Because ETS focuses on the overall success of the student to complete middle school and high school, student grades will be checked throughout the year. Students, who have below a 2.0 GPA and/or a letter grade of “D” or “F” in any single class, will be put on probation and recommended for ETS tutoring. Students will be placed on probation until the end of his/her semester, at which time his/her grades will be reassessed. Failure to bring grades up may result in student being dropped from the ETS program. However, if the student attends tutoring and is making an attempt to improve his/her grade, probation may be extended another semester/trimester. The following schedule will be used to check grades:
Please take time to fill out this entire Student Intake Application. Both parent/guardian and student must sign this packet to be considered complete. ETS is a federally funded program and a social security number is required to have on file for all ETS participants. If you do not feel comfortable providing a social security number on this form, please contact the ETS office.
