I, the parent/guardian of the Upward Bound applicant, certify that the information reported above is accurate and complete to the best of my knowledge. I agree to provide documentation to verify the information reported, including a copy of my 2023 Federal Income Tax Return or proof of other forms of income if my student is accepted to the Weber State University Upward Bound program.

I hereby give my permission for my student’s school and district to release grades, transcripts, test scores, and any other information needed to provide educational services to my student to the Upward Bound program at Weber State University.

If my student is accepted to the program, I also give my permission to Weber State University Upward Bound personnel to view and obtain Weber State University transcripts, schedules, test scores, and financial aid records from Weber State University while enrolled as a college student and for Concurrent Enrollment credit while in high school.

I understand that this information will be kept in a locked, confidential location and will be viewed and used by authorized Upward Bound staff members only, in compliance with FERPA.
