Clovis Community College Center-SSS-Application Form

Student Support Services (SSS) is a free federal TRIO Program that serve students who are first generation, low-income, or have a disability. These programs provide academic support and guidance to help Clovis Community College students meet the challenges of obtaining an AA/AS degree, AA-T/AS-T degree and/or transfer to a four-year university. If you are committed to achieving success, SSS can help you achieve your academic and career goals.

Minimum Qualifications:
- U.S. Citizen or permanent resident and meeting residency requirements for Federal Financial Aid or California Dream Act
- Enrolled in 12 units or more in an AA/AS, or Transfer Program
- Be in good academic standing and have a 2.0 or better
- Have less than 50 college units completed
- One or more of the following apply
  - Be considered low income
  - Parents or guardians did not complete a bachelor’s degree or higher
  - Have a disability
Documents required in order for processing your application:
- Application
- Financial Aid SAR, Award Letter, or verification of income
- Educational Questionnaire
- Transcripts
  - High School Transcripts for first-time students and/or All other colleges previously attended
- Recommendation Form
The U.S. Department of Education provides TRIO funding for the SSS Grant. Consideration of applicants is without regard to race, color, religion, national origin, sexual orientation, marital status, or disability. Personal information collected is for reporting requirements and is confidential.

Student Responsibility Agreement

If selected to participate in the SSS program, I agree to the following statements (please initial)*

Meet with an SSS counselor two times every semester
Attend orientation at the beginning of each semester
Be enrolled in 12 units or more during Fall and Spring
Maintain 2.0 GPA or higher every semester
Be pursuing an AA/AS or Transfer to a four year
Participate in events and workshops provided by the SSS Program
Utilize the library and Tutorial Learning Center
Inform SSS Counselor of address, phone, or class schedule changes
Allow communication with instructors to monitor academic progress
