Client Statement of Truth and Permission - All statements made are true and complete to the best of my knowledge.  I understand that the above information will be entered into the Educational Opportunity Center/KCAC/KentuckianaWorks' database and used for statistical purposes (to funders and other entities) and follow up (enrollment) activities.  I give KCAC/KentuckianaWorks (and any of its affiliate entities) permission to give my personal information to educational institutions and affiliated agencies.   KCAC has my permission to retrieve information from secondary schools, postsecondary schools, and other agencies regarding my academic interest, academic status (admissions, enrollment), academic record (grades, test scores), financial aid award and/or financial aid status throughout my post-secondary career.  I agree to release KCAC/KentuckianaWorks and its entities from any liability arising from services rendered by KCAC/KentuckianaWorks or its use of dissemination of the above information.
