Congratulations UBMS Senior! You have achieved the goal of completing high school. Now it’s time to look forward to the future of a college student. The Summer Bridge Program is here to help you with a smooth transition from high school to college life. The program provides tuition (up to six credit hours of approved courses: Math, English or Science), textbooks and supplies, as well as, tutoring, mentoring and monitoring by UBMS staff.

In order to participate in the Summer Bridge Program, you must fulfill the following requirements:
1. Must graduate from high school with a Core cumulative G.P.A 2.0 or better on a 4.0 scale.
2. Must have an ACT composite score of at least 20, SAT score of at least 1020, Math component score of 19, and/or English component score of 18.
3. Must have an 85% attendance rate during the 2019-2020 academic year. This include Saturdays, all senior workshops and optional activities (college/culture tours, etc.).
4. Must have completed all senior year requirements by the required deadlines.
5. Must agree to adhere to the Upward Bound Math/Science (UBMS) Code of Conduct.
6. Must complete a Southern University at New Orleans (SUNO) application for admission and have been accepted for the summer semester.
7. Must agree to the rules, regulations and laws set forth by Southern University at New Orleans.
8. Must agree to achieve a grade of “C” or better in each SUNO course to complete the Summer Bridge Program.
9. Must complete the following application in its entirety and return it to an Upward Bound Math/Science Academic Advisor or the Upward Bound Math/Science Office.
Note: You do not have to continue your education at SUNO after the summer semester. You may transfer your credits to the school of your choice.

I also understand that if the terms of this agreement not met, I will not receive assistance with the services associated with the Summer Bridge program including tuition, meals, textbook fees, supplies or any additional fees. ALL FEES WILL BE THE RESPONSIBILITY OF THE STUDENT IF THE STUDENT RECEIVE A GRADE OF “F” OR IF THE STUDENT WITHDRAW.

Records Release Authorization
According to the Department of Education, Upward Bound Math/Science program is required to track participants’ post-secondary education for the next 6 years. I hereby consent to the release of my college/university records (transcripts, final transcripts and other information regarding my post-secondary education performance) to Southern University at New Orleans Upward Bound Math/Science Program. This release is effective throughout my post-secondary education journey and is effective for any university/college I attend for the next 6 years.
