    • I give my consent to Student Support Services Program to secure and review information that may be needed to verify my income, to complete applications to postsecondary institutions and financial aid programs. I hereby grant permission to the personnel of my child's school to provide transcripts, test scores, and academic progress reports to the Student Support Services Staff. I certify that the above information is correct to the best of my knowledge. I understand that all the information will be held in the strictest of confidence and used for statistical purposes only.

    • Student Support Services Program (SSSP) provides supplementary academic services to eligible Norco College students (see Program Eligibility). The services of the program are highly personalized and the program is limited to 160 students. Applications are available year-round as space permits. The goal of SSSP is to increase the college retention and graduation rates of its participants and help students make the transition from one level of higher education to the next. The SSS program is funded $231,103 on an annual base by a grant from the U.S Department of Education.

      I grant permission to Norco College, its employees and agents, to take and use visual/audio images of me. Visual/audio images are any type of recording, including but not limited to photographs, digital images, drawings, renderings, voices, sounds, video recordings, audio clips or accompanying written descriptions. Norco College will not materially alter the original images. I agree that Norco College owns the images and all rights related to them. The images may be used in any manner of media without notifying me, such as college-sponsored web sites, publications, promotions, broadcasts, advertisements, posters and theater slides, as well as for non– college uses. I waive the right to inspect or approve the finishing images or any printed or electronic matter that may be used with them, or to be compensated for them. I release Norco College and its employees and agents, including any firm authorized to publish and/or distribute a finished product containing the images, from any claims, damages or liability, which I may ever have connection with the taking of use of the images or printed material used with the images.

      I am at least 18 years of age and competent to sign this release. I have read this release before signing, I understand its contents, meaning and impact and I freely accept the terms.

    • Field trips or excursions are for the purpose of education, cultural, and social enrichment. Norco College, the Riverside Community College District, the Board of Trustees, and College Employees or agents and the sureties shall not be held responsible under any circumstances whatsoever by the undersigned, his state, or heirs for any injury. Damage, or loss to the person which the undersigned uncured while attending the off-campus functions.

      State of California Educational Code #72640 states: All persons making a field trip or excursion shall be deemed to have waived all claims against the district, or the State of California for injury, accident, illness, or death occurring during or by any reason of the field trip or excursion.

      It is imperative that all participants observe the regulations set forth by our colleges and California Educational Code.

      • Riverside Community College District and the California State Education Code prohibit the consumption of alcoholic beverages during a college function regardless if you are 21 years old or over.
      • The use of an illegal substance is a violation of state and district regulations.
      • Destruction of any property is a violation of state law and district regulations. Any cost associated with such destruction is the responsibility of those involved.
      • Students are expected to attend all agenda events unless they have a valid excuse, which must be pre-approved by the advisor, coach or instructor.
      • After scheduled events, as outlined by the written agenda, are completed, the student will not be under the immediate and/or direct supervision of an employee of such district or board.
      • Any violation of the rules may result in exclusion from the future field trips, excursions and in severe cases; it may result in suspension or expulsion from the college.

      Your signature on this document indicated that you have read the document, understand its expectation and liability limitations that you are of legal age to sign the waiver, and do so voluntarily.

    • Medical Insurance Information

    • In the event of any medical emergency, I grant to the college or any of its representatives the full authority to take action deemed necessary to protect my health and safety at my expense, including but not limited to placing the Participant under the care of a doctor or in a hospital at any place for medical examination and/or treatment, or returning the Participant to their home city at his or her expense if such return is deemed necessary after consultation with medical authorities.
