    • Student Permission & Participation Agreement 
    • TRIO-Student Support Services (SSS) at Marian University is a federally-funded educational program that provides services to help students earn a bachelor’s degree. Students selected to participate in the TRIO-SSS program receive individualized assistance in navigating their path through Marian University and to graduation.

      By providing information requested on this form, you will be notified of special academic services and financial resources for which you may be eligible. This form must be completed and returned to be considered for services.

      TRIO-Student Support Services (SSS) staff select students to participate based on the following criteria:

      • are first-generation college students (neither parent completed a bachelor’s degree); or
      • are from a family earning a limited income (determined by taxable income level and family size); or
      • have a documented physical, psychological or learning disability (that may affect your role as a student).

    • THIS FORM IS NOT RELATED TO ANY APPLICATION FOR FINANCIAL AID

      This section must be completed and signed by the Head of the Household* and is not related to any application for financial aid.

      *The applying student is considered the Head of the Household only if they are classified as “independent” according to federal standards which typically means age 24 or older. Students may be considered “dependent” even if they live separate from their parents and/or are responsible for their own educational bills and expenses.

      Use 2020 Federal Income Tax Information (For verification with Federal TRIO student aid income levels effective Jan. 13, 2021.)

    • Please tell us about your family:

    • Parent/Guardian Section (This section must be completed and signed by a parent/guardian!) 
    • For dependent students (usually under 24 years old), this section must be completed and signed by a parent/guardian. Independent students (students who are married, have children, or are 24 years of age or older) can complete this section on their own. List taxable income (as reported on line 15 of IRS Form 1040) to help us determine income eligibility. Please verify that the amount you enter matches the same amount listed on your tax form.
