The following information is needed to evaluate your application for acceptance into SSS and to help determine which services you need. The information you provide will be treated confidentially.

Documentation must be provided by an appropriate Professional or State Rehabilitation Office.

Federal regulations require income information to determine which applicants meet TRIO-SSS federal income eligibility guidelines. Income eligibility is determined by your NUMBER OF HOUSEHOLD MEMBERS/FAMILY UNIT (from your FAFSA, Line 72) and your family's 2020 TAXABLE INCOME (from your 1040 Income Tax form, Line 15). PLEASE NOTE: Your TAXABLE INCOME should be less than your ADJUSTED GROSS INCOME.!

A dependent student is under the age of 24, single, with no dependents, and no military service.

I certify that the information that I have provided on this SSS application is correct and complete. By signing this form, I authorize TRIO Student Support Services to obtain any and all information and documentation necessary for processing and the ongoing evaluation of my academic pursuits. I also give my permission for the TRIO Student Support Services staff to share my information (including but not limited to IEP, 504, Test Scores, FAFSA) with other staff, faculty, and mentors if it will assist in my academic success.

The term "income eligible individual" means an individual whose family's taxable income for the preceding year did not exceed 150 percent of the poverty level amount.
