LAST YEAR’S TAXABLE INCOME

Vet-SSS is a federally funded program that requires verification of every participant’s income. Please check your TAXABLE income for the previous year (line 43 on 1040 form, line 27 on 1040A form, line 6 on 1040EZ form). This information may be helpful in notifying the student about financial aid, scholarships, and other special programs, but does not affect acceptance.

Student Acknowledgement:

1. I understand that I must meet with my Vet-SSS Success Coach a minimum of twice persemester.
2. I understand that I must maintain a cumulative GPA of 2.0 or better and if placed on academic probation, I must meet with my Vet-SSS Success Coach four times during the semester and attend skill building workshops & tutoring until no longer on academic probation.
3. I understand that I must attend all five days of Warrior Reboot in August preceding the start of the Fall semester (incoming students only).
4. I understand that I must attend tutoring and group student sessions throughout my freshman and sophomore years, or as assigned by my Vet-SSS Success Coach.
5. I understand that I must register in accordance with my primary advisor & Vet-SSS Success Coach’s recommendations.
6. I understand that during my senior year I will meet with my primary advisor to conduct a degree audit, and with my Vet-SSS Success Coach to conduct an exit interview from the Vet-SSS program.
