UPWARD BOUND RURAL

The Student has my permission to attend Upward Bound events such as instructional days, campus tours, volunteer activities, etc. I understand that my son/daughter will be chaperoned on this event by Upward Bound Staff. In consideration for allowing my child to participate in these events, we, acting for ourselves and our child, release and agree not to assert a claim in the future against the TRIO Programs, Coastal Bend College, and Beeville, Mathis, Pettus, Sinton, Odem or George West I.S.D. arising out of my son’s/daughter’s participation in the Upward Bound event. We further expressly agree to indemnify the TRIO Programs, Coastal Bend College, and Beeville, Mathis, Pettus, Sinton, Odem, George West I.S.D. from any and all claims that may be brought against them by anyone for the purpose of enforcing a claim for damages sustained by my son/daughter while participating in this event. I understand that photographs/videos will be made during the activities and I give my permission for TRIO to utilize these photographs/videos for TRIO publicity.

Consent for Treatment of a Minor

I, the undersigned, as the parent or legal guardian of a minor child, {main_name} hereby authorize diagnostic medical and/or surgical treatment on my child as may be deemed necessary in order to assure the safety of my child. It is distinctly agreed and understood that the attending physician and appropriate staff shall not be responsible in any way for any consequences from said diagnostic, medical and/or surgical treatment and is fully released from any and all claims and demands whatsoever which arise, grow out of or be incident to such
diagnosis, treatment or surgery insofar as the law allows and provided that these services are performed with ordinary care and to the best of their ability.
