• 2024-2025 GECAC UPWARD BOUND PROGRAM application

  • The Upward Bound Program of the Greater Erie Community Action Committee (GECAC) in collaboration with Penn West Edinboro University of Pennsylvania is designed to provide an opportunity for eligible high school students to succeed in their overall high school performance and ultimately pursue higher education. The mission of the program is to increase the rates at which participants enroll in and graduate from institutions of post-secondary education. Upward Bound is a federally funded TRIO program that provides all services FREE to eligible students.

    • THE UPWARD BOUND PROGRAM IS A FEDERALLY FUNDED PROGRAM. FOR THE APPLICANT TO QUALIFY, VERIFICATION OF INCOME IS NECESSARY. ALL INFORMATION IS CONFIDENTIAL AND FOR PROGRAM USE ONLY

      Program eligibility is determined and partially based on, ANNUAL TAXABLE INCOME. Please indicate the level of your TOTAL FAMILY TAXABLE INCOME FROM LAST YEAR (refer to IRS Form 1040 ~ line 43, 1040A ~ line 27, 1040EZ ~ line 6.) Note: this is not the same as your adjusted gross income_ please use TAXABLE INCOME.

    • Your family may be eligible for additional GECAC services. Please provide the following information for all other individuals living in the household. Please use additional sheet for more than 6 members.

    • PART 5: PARENT COMMITMENT / RECORDS RELEASE We understand that, if selected, our son / daughter will be required to attend and participate in Upward Bound meetings, activities and field trips on an ongoing basis, including two Saturday Sessions a month during the academic year. He/she will maintain at least a 2.5 GPA in school and will also attend tutoring classes, if needed until his /her graduation from high school. If possible, he / she will participate in our 6 week summer academy at least one summer during his/her time in the Upward Bound Program. This application remains in effect until my son / daughter and I notify the UB Program Director in writing, that he / she no longer wishes to participate in the Upward Bound Program.

    • By signing this release form, I agree to allow the Greater Erie Community Action Committee Upward Bound program to obtain academic records, i.e. grade reports, transcripts enrollment information, IEP’s, test scores and any other relevant educational information for purposes of program evaluation from any high school or post-secondary institution that I attend. This information will be handled in strict confidence according to federal regulations.
