Directions:

1. REVIEW the Program Service options below. Online via virtual formats are available; options shown with # will be available when in-person services can be offered. 
2. CHECK the box for each of the services that you would like your TBED student to receive.
3. Then put the points for the service in the POINTS column.  
4. TOTAL the points for all of the services you have selected; 40 max. points for Oct. 1, 2022  –  Sept. 30, 2023 grant year. 
5. SIGN and REVIEW the form to the Tunica-Biloxi Education Department.

Please note:  This table references the cost value range of service points funded by TBED.  All Student Services provided through TBED are free to eligible Tunica-Biloxi students.

A Check-Out Agreement is required for use of all items in this section.

You may select:

• 1 Tablet or 1 Laptop, not both
• 1 Calculator, not both

You may maintain your check-out of any item previously selected by selecting the check-out continuation option; students will not be issued multiple devices.

We certify that up to 40 points in program services have been requested for the above-named TBED student to receive during the Oct. 1, 2022  –  Sept. 30, 2023 grant year.  We understand that options shown in gray will be available when in-person services can be offered. We also understand that TBED will do the best to accommodate our service requests but reserve the right to amend and/or modify selections based upon the date of enrollment in TBED, and accessibility of services and providers.
