Dear Applicant:
Thank you for your interest in the Upward Bound Math and Science Program with CARE at FSU. Upward Bound Math and Science (UBMS) is a federally funded program for 9th through 12th grade students to develop skills and gain experiences necessary to realize their college and career opportunities related but not exclusive to science, technology, engineering and math. Participants in the program through the Center for Academic Retention and Enhancement at Florida State will have access to: a) tutoring, academic skill development and course readiness, b) social and cultural activities, c) college and career awareness activities and d) academic and personal coaching.
Prior to beginning the Upward Bound Math and Science application, please take a moment to review the checklist below of required documentation. Students that have a favorable review of their application materials will also be invited for an interview with program staff.

- Confidential Financial Information and Certification Statement – Emailed after this form submitted
- Parent’s signed 2023 Tax Form (1040, 1040A, 1040 EZ) or Alternative Financial Statement with documentation – Uploaded with financial information form
- • Unofficial high school transcript (9th grade applicants, please submit your current report card) – Emailed to jmcnealy@fsu.edu
