AAMU TRiO UPWARD BOUND PARTCIPANT & PARENT(S)/GUARDIAN(S) AGREES TO:

· Understanding the function and purpose of the TRiO Upward Bound Program, I will take advantage of all service available.

· Attend most scheduled Weekend Session & tutoring as needed or recommended.

· Give a copy of every report card and progress report to the TRiO-Upward Staff for review each reporting period.

· Attend school regularly

· Attend culture and enrichment activities, career exploration, seminars, and workshops conducted by the TRiO-Upward Bound thought out the academic and residential component.

· I understand the goals, objective, and requirements of the TRiO-Upward Bound Program. I also understand that if my child does not fulfill the required goals and objective, he/she may be TERMINATED from the program. I certify that all of the information is factual and correct to the best of my knowledg
