Income Verification - Parent

 

Documentation of household size and taxable income is required to determine financial eligibility for participation in the Talent Search Program.  Please complete the income verification section below. Please list your taxable income (not salary or gross income).

If you did not file taxes, please select less than $23,475.

STUDENT & PARENT CONTRACT

I agree to participate in Talent Search activities, communicate with my
Educational Specialist & consider the advice they provide as I explore career & educational opportunities. Any participation in the program may be photographed, filmed and used in any media format for the purpose of student outreach or promotional materials.

We understand that our child’s Educational Specialist may be provided with information from his/her school regarding their academic progress (FAFSA reports, scholarships, college acceptance/denial letters, ACT/SAT scores, report cards, class schedules, transcripts, etc.).  We understand that we are assisting our child in meeting their academic goals by providing this information.

The Talent Search program standard is for its students to maintain at least a 2.5 GPA. Failure to participate regularly in the program or to give sufficient effort towards my academic success may be grounds for termination from the Talent Search program.
