    The TRIO Educational Talent Search Program is a 100% federally funded program by the US Department of Education, designed to encourage students in grades 6-12 to complete secondary school and explore college and career opportunities.


    The ultimate goal of the ETS Program is to assure that our participants graduate from high school, apply and enter the college of their choice, secure financial aid/scholarships to pay for their college education, and graduate with a postsecondary educational credential.

      *Neither parent has received a 4-year degree
      Qualify as Low Income and/or Potential *First Generation College Graduate•
      OR Live in Indian River County
      Be at least 11 years of age and grades 6-12
      Attend Sebastian River MS, Gifford MS, Oslo MS, Sebastian River HS, or Vero Beach HS
      *A copy of permanent residency card must be attached

      1. Complete all sections of the Application
      (pages 1-5), and Teacher Recommendation
      Form (if applicable).
      2. Turn in to your school counselor.

    • As a participant in the Educational Talent Search Program, I agree to the following:

      ➢ EXPECTATIONS

      o Students are expected to be present and on time for all workshops, field trips and individual appointments scheduled by their ETS Academic Advisor.
      o Students are expected to have a positive attitude and act in a respectful manner that is neither disruptive nor rude during all meetings, workshops and field trips.
      o Students are expected to actively participate in all meetings, workshops and field trips.
      o Students are expected to give their ETS Advisor a copy of all six-week and semester grade reports.
      o Students are expected to encourage their parent(s) and/or guardian(s) to attend the Educational
      Talent Search Parent Meetings and participate in ETS activities.

      ➢ DRESS CODE

      o Sagging pants or anything that reveals underwear or demonstrates indecency is not permitted.
      o The length of shorts should be at least to the end of the student’s fingertips.
      o All clothing holes or tears should not be past mid-thighs.
      o Tube tops, halter tops, tank tops that do not cover the shoulders and/or mid-drift area, and sheer or see-through items are not permitted.
      o Items displaying profanity, offensive symbols, offensive language, or a visible message across the bottom are not permitted.
      o Hats or caps should be removed when inside buildings.
      o Students MUST wear their EDUCATIONAL TALENT SEARCH T-SHIRTS during program activities. i.e., Field trips

      ➢ FIELD TRIPS

      o All students are required to submit a Field Trip Consent Form and other required forms, signed by a parent/legal guardian to attend field trips.
      o All students will be expected to arrive on time for all scheduled activities as outlined in the itinerary provided and instructed by ETS staff.
      o All students will be expected to always remain with assigned ETS staff during group tours and activities.
      o All students are expected to be picked up promptly upon return from a scheduled activity.


      I understand and agree to accept the duties and responsibilities outlined above. I understand that failure to adhere to any
      of the above-mentioned conditions may result in being placed on probation or suspension from the Educational
      Talent Search Program.

    • MEDICAL CONSENT ACTIVITY AND FIELD TRIP AUTHORIZATION 
    • MEDICAL CONSENT/ ACTIVITY AND FIELD TRIP AUTHORIZATION
      As the parent/guardian of the applicant I do willingly execute this release in consideration of the educational benefit to be derived by my child in participating in Educational Talent Search, a College sponsored activity.


      I do hereby release, discharge, and hold harmless Indian River State College, its agents and employees, from any and all liability and claim either we or our child may suffer as a result of these requests for medication and emergency treatment. Also, any such causes of action which might be brought by my child for loss of property, personal injury or death sustained by my child arising out of travel or activity conducted by or under the control of the College. I understand that the term “College” as used herein shall include the employees, administrators, agents, and Board of Trustees of Indian River State College.


      I understand that every effort will be made to contact parents/guardians in the event of emergency. In an emergency requiring medical care and treatment, I hereby authorize any physician, hospital or other healthcare provider to give such care to my child. I also hereby give permission for the transport to/from a doctor and/or hospital by college staff or ambulance. If medical services are needed by my child while participating in the Educational Talent Search program or on field trips, I give my permission for my child to receive any medical services deemed necessary.

      MEDICATION AUTHORIZATION
      Prescription and non-prescription medication for full-day and overnight field trips must be supplied by the parent/guardian in the ORIGINAL CONTAINER and given to the program director. Only emergency medications will be carried on half-day field trips. Students may carry certain medications (ex. EpiPen, inhalers, insulin) if the program director is informed that the student needs and is carrying the medication.

      (A SEPARATE MEDICATION FORM MUST BE COMPLETED BY PARENT BEFORE TRIP)
      Treatment for your child, may include:

      2. Treatment of healthcare problems (stomachaches, earaches, headaches, cuts, sores, colds, coughs, ringworms, etc.)

    • RELEASE & AUTHORIZATION 
    • RELEASE OF ACADEMIC RECORDS AND INFORMATION
      I authorize the release of school records for the applicant’s file that may be requested by Educational Talent Search at Indian River State College. I understand that the U.S. Department of Education funds Educational Talent Search and will use these records to provide academic advisement and other services. I also understand that these
      records will be handled in a confidential manner and that they will be made available only to program staff and representatives from Federal and State Departments of Education.


      I release the following information from public school records to the Educational Talent Search program: transcripts, test scores (TABE, PERT, ACT/SAT, Academic History, etc.), grades and progress reports, information concerning disciplinary actions, attendance record 6 th-12th, income and other information necessary to the
      program for application completion, to provide academic support, and verification of student progress.


      All information provided on this form will be kept confidential and will not be disclosed to a third party by the program without my permission. I understand that my parents/guardians will be required to disclose information, such as household income, necessary to determine my eligibility for program participation.
      Note: A photocopy of this record release form should be accepted as an original and the date of the application has no bearing when the Indian River State College Educational Talent Search Program requests the information.


      FOCUS PORTAL RELEASE
      I agree to establish a Parent Portal Account with the School District of Indian River County. I understand that I must complete the Release or Transfer of Student Data form allowing Educational Talent Search program to access my child’s grades. I understand that Educational Talent Search program collects my child’s grades, attendance, and test scores needed for grants and tracking purposes. I agree that failure to complete this form may result in my child being terminated from the program.


      PHOTO RELEASE
      I hereby grant Educational Talent Search at Indian River State College permission to photograph/video my child and/or to use their likeness in photograph(s)/video in any and all of its publications and in any and all other media, whether now known or hereafter existing, controlled by Educational Talent Search at Indian River State College, in perpetuity, and for other uses by the College or Program. I will make no monetary or other claim against Educational Talent Search at Indian River State College for the use of the photograph(s)/video.


      COMPUTER USAGE
      I grant permission for my child to access networked computer services such as the Internet, World Wide Web, and electronic mail.


      By signing, I hereby acknowledge and certify that the information contained in this apllication, and any attached supporting documentation provided, is true and correct.
