{main_name} has my permission to attend with Talent Search to the June 2025 summer events event on {pleaseSelect}. I understand that my son/daughter will be chaperoned on these events by the TRIO Talent Search. In consideration for allowing my child to participate in these events, we, acting for ourselves and our child, release and agree not to assert a claim which we may in the future against TRIO Talent Search Program, Coastal Bend College, and the I.S.D. arising out of my son’s/daughter’s participation in the campus tour event. We further expressly agree to indemnify the TRIO Talent Search Program, Coastal Bend College, and the I.S.D. from any and all claims that may be brought against them by anyone for the purpose of enforcing a claim for damages sustained by my son/daughter while participating in this event. I understand that photographs/videos will be made during the activities and I give my permission for TRIO Talent Search to utilize these photographs/videos for Talent Search publicity.
I am also granting permission for my child to travel to and/or from the above mentioned TRIO Talent Search sponsored activity in a vehicle or vehicles not owned or operated by the child’s school district which include but are not limited to official Coastal Bend College vehicles and vehicles rented by the TRIO Talent Search program.
Consent for Treatment of a Minor
I, the undersigned, as the parent or legal guardian of a minor child, hereby authorize diagnostic medical and/or surgical treatment on my child as may be deemed necessary in order to assure the safety of my child. It is distinctly agreed and understood that the attending physician and appropriate staff shall not be responsible in any way for any consequences from said diagnostic, medical and/or surgical treatment and is fully released from any and all claims and demands whatsoever which arise, grow out of or be incident to such diagnosis, treatment or surgery insofar as the law allows and provided that these services are performed with ordinary care and to the best of their ability.
BEHAVIOR AGREEMENT FOR STUDENT
I understand that the rules I am held responsible to at my school campus are the same rules that apply during any event/activity with the Talent Search Program. Fighting, obscene language, threatening others, theft and damaging property is strictly prohibited. Alcoholic beverages, controlled substances, and weapons are strictly prohibited. Appropriate clothing is to be worn at all times. Students must show respect for TRIO Talent Search staff at all times. If any rule is broken I understand, if it becomes necessary, the Talent Search staff will call my parents and they will be required to pick me up IMMEDIATELY, at their own expense. I also understand that failure to abide by any of the recognized rules may result in my dismissal from the Talent Search Program.
