Waiver of Liability
I voluntarily agree to assume all risks of participating in the events and accept sole responsibility for any injury to my student, myself, and my family (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim liability, or expense, that we may experience or incur in connection with my student’s attendance at events (“Claims”) during the Academic Year events 2024-2025 in which the program(s) may participate. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Tusculum University and UPWARD BOUND/COCKE HAWKINS UPWARD BOUND/UPWARD BOUND MATH & SCIENCE, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind related to the above named event. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Tusculum University and/or UPWARD BOUND/COCKE HAWKINS UPWARD BOUND/UPWARD BOUND MATH & SCIENCE, its employees, agents, and representatives during or after participation in any University program.
