TRIO Talent Search Program Application

I hereby certify that all information submitted on this form is accurate to the best of my knowledge. I also give permission for Palm Beach State College-Talent Search Program to have access to any school records of the student listed on the application. SIGNATURES MUST BE IN CURSIVE WRITING AND INK

To participate in the Talent Search program at Palm Beach State College, I agree to attend school regularly, maintain good behavior, and avoid any disciplinary issues. I will strive to earn high grades in all my classes and notify my TS Postsecondary Advisor if there are any changes to my class schedule, home address, or phone number or if my family plans to move. If I face any academic or personal challenges, I will seek help when needed. I also agree to participate in special program activities, including college visits, financial aid workshops, and cultural events. I will work to be successfully promoted to the next grade each year, apply for financial aid during my senior year, and graduate from high school with the intention of enrolling in a post-secondary education institution. Through these commitments, I aim to achieve my academic and career goals.

Records will be used to assess student needs, monitor progress, document eligibility, and for recording purposes. This information is confidential and protected under the General Education Provision Act. Confidential information will not be released without your written consent. I hereby grant permission for the TRIO Talent Search Program at Palm Beach State College to have access to educational records, including but not limited to grades, test scores, free/reduced-priced lunch eligibility, class schedules, postsecondary enrollment, discipline records and attendance for the duration of the student’s enrollment in Talent Search. This also authorizes postsecondary institutions to release copies of college academic enrollment and student aid information to the PBSC Talent Search program. This also authorizes Talent Search advisors to access my child’s Student Information System.

Photographs of our participants may be included on our website or possibly other college-approved media. We require your authorization to use any photos we
take of your student in order to do this.
Please read and sign if you agree to the following terms.
I hereby irrevocably consent to and authorize the use and reproduction by Palm Beach State College, or any person or organization authorized by Palm Beach State College, of any and all photographs, videotapes, or film which have been taken of my student, both singularly and as part of a group, without any compensation to me. All positives and negatives shall constitute solely the property of Palm Beach State College.

The undersigned parent or legal guardian of the student who is currently enrolled in High/Middle School hereby grants permission for my student to participate in activities sponsored by Palm Beach State College’s Talent Search Program.
The undersigned does further join in the release of Palm Beach State College, its trustees, officers, employees, and agents from any and all liability or claims for
losses or damages that the student may sustain while participating in travel activities sponsored by the Palm Beach State College’s Talent Search Program.
The undersigned expressly understands and agrees that Palm Beach State College, its trustees, officers, employees, and agents assume no liability for the student’s participation in travel activities. The undersigned grants authority to Palm Beach State College’s representative to authorize medical treatment as the treating or consulting physician deems to be necessary while participating in the college’s travel activities. Therefore, the undersigned releases PBSC, its trustees, officers, employees, and agents from any and all liability or claims for damages arising from exercising the authority granted.

Should my student become ill or be involved in an accident while participating in PBSC Talent Search-sponsored activities, PBSC’s representative will make every attempt possible to notify me of circumstances of the incident. If PBSC cannot reach me immediately, or if the situation is such that it warrants immediat medical attention, I grant PBSC’s representative authorization to secure medical treatment for my student. It will be my responsibility to assume payment for such medical treatment
