    • Cleveland Community College TRIO SSS will retrieve information you have on file with the CCC Financial Office. You may be contacted to verify income eligibility.

    • Academic Planning & Program Participation 

    • If accepted to SSS, I agree to these participation requirements: (To acknowledge your understanding and acceptance, please select ALL requirements.)

      • Attend at least two meetings per semester with my assigned Advisor.
      • Keep my assigned TRIO Advisor informed of my progress via personal contact throughout the semester. Appointments are highly encouraged.
      • Contact my assigned TRIO Advisor to explore my options and to do a financial aid verification before withdrawing from a course.
      • Demonstrate a commitment to graduate and/or transfer by enrolling in courses that are required or recommended for my program of study.
      • Maintain at least a 2.0 cumulative (overall) grade point average.
      • Participate in at least one TRIO-SSS activity each semester that I am registered at CCC. Example activities include, but are not limited to workshops, tutoring, college tours, cultural events, or other TRIO-SSS sponsored activities.

      If accepted to SSS, I authorize SSS to take photographs of me to be copyrighted, published, exhibited, or included on the official Cleveland Community College website and/or other publications. I further release Cleveland Community College from any and all claims for damages for libel, slander, invasion of the right of privacy or any other claim based on the use of said materials. If I would like to be excluded from any photos taken, I will politely let the SSS staff know.

      I certify that all the above information is correct. I release the following information to Cleveland Community College TRIO Student Support Services: High School transcripts, assessment scores, grade reports, financial aid records, and any other information from my records that pertains to my enrollment. I also authorize CCC TRIO Student Support Services to release information from my records for the purpose of providing services that I might need.
